Consumer Direct Personal Care (CDPC) established self-directed care services in New Mexico in 2002.  The services are specifically intended to allow individuals with health care needs to remain in their homes and communities and to avoid entering a long-term care facility.

Self-directed care is designed for individuals who are capable of directing their own personal care services or appointing a Personal Representative to act on their behalf. 

Service requirements include:

• The need for assistance with activities of daily
  living such as:
  

  - medication reminders	- transfers
  - bathing & hygiene	- dressing & grooming
  - ambulation	- meal preparation and eating
  - cognitive functioning	- household & support services
  - toileting, bowel & bladder care

• Completion of an assessment for placement in our services.
